FAPA you are the Best! I visit with a friend Michael Padgett and he was very Happy with his stay here! He mention that he had a great CNA that made sure he was taken care of.He came in not eating but he said the food is great.He wanted to mention his Therapist Marcus and Molly is Wonderful! Thanks to your staff for making his stay Great!

My stay here has been good! I think that the call bell could be a little faster, but overall my stay is good. Therapy has been great! I look forward to going to therapy because I need it. The nurses have been great! Housekeeping is friendly and keeps my room clean. Both of my social workers have been very helpful. Ms.Nora has been fantastic! And she doesn’t mind sharing her time. I hope I don’t have to come back but if I did I would come but hopefully you all could invest in better beds. Thanks for everything!

This place is disappointing to me because each visit my family room is filthy housekeeping is very poor his belongings is disorganized no updates with his appointments or medication

Therapy was constantly helpful and considerate. Marcus, Molly and Toni .Val was helpful. Dr. Nora listened to my concerns and questions. Justin is always kind and considerate. The cleaning staff was very helpful including JoJo. Ms Sonji was very helpful and kind. For the most part the nursing staff was attentive and helpful.

MY STAY AT FOREST ACRES POST ACUTE HAS BEEN GREAT.EVERYONE HERE HAS BEEN FRIENDLY AND HELPFUL.MY THERAPIST MARCUS,SHARON,KELLYI AND DIANE IS AWESOME.THE FOOD IS TASTY AND GREAT.MY NURSE LAYA AND LEXI WAS THE BEST.MY CNA DEMARRIE WAS GREAT AND WAS HELPFUL WITH A SMILE EVERYTIME. DR .LATONYA WAS ALWAYS AVAILABLE AND STOPPED BY DAILY WITH A SMILE. I WOULD REFER OTHERS TO THIS WONDERFUL FACILITY AND I WOULD RETURN IF I HAD TOO.THANKS EVERYONE FOR A JOB WELL DONE.

I am enjoying my stay here at FAPA and I would like to share my experience that I have had with some of the workers here. Justi. Is a wonderful CNA, he really shows how much he cares about his residemrs.He has taken great care of me.Cindy my night CNA is great as well. She always checks in on me. My nurse Cierra is friendly and always nice to me. Destiny and Brittney are great housekeepers and they are always nice and keep my room clean. Sharon in Dietary has been nice to me and make sure I am good. She makes sure I am happy. Nora is the Best. She is honest and cares about her job. It is great when you run into people that CARES. Thanks to all of you that went the EXTRA MILE

We had an awesome time doing Activities with the resident. You can tell the facility takes care of their residents by providing interactive activities to keep their morale high. We played games like Bingo, Uno, Spades, and Five Second Rule while painting residents nails at the nail station. It was a fun day at Forest Acres. Keep up the good work!

I am writing on behalf of my friend Patricia Galloway. I was visiting with her and she was so happy with this facility! She was telling me how everyone treats her so good! She said the nursing staff is wonderful and they makes sure she is taken care of.I visit with her after she had finish playing BINGO and this activity makes her day! Please keep the BiNGO going! As I was leaving she was about to eat and she had Fried Chicken! I was just happy to see her as happy as she was! FAPA keep making your patients HAPPY.

I have been here for about 4 years. I wanted to say that I enjoy where I am. Everyone here makes me feel like I am at home. My CNA Fatisha is great. She makes sure I am taking care of when she is here. My nurse Melissa is a blessing! She is always on top of things to get me right. My night nurse Rochelle was on it at night but she is now the wound nurse and I miss her so much. Marcus in Therapy was a blessing when I had him. Alvin the maintenance guy keeps me right whenever I need him. Meka my housekeeper keeps my room clean and smelling good. Social services helps me when I need them. Sonji the receptionist checks on me daily, And I appreciate her. Gloria and her assistant in activities pops in and let’s me know what’s going on in activities. Sharon in dietary makes sure I eat and gets my snack ready for dialysis. Her staff comes and ask if I need anything. Ahri in the kitchen is amazing she always takes my menu and makes sure it’s right. This is a beautiful place to come if you need to or your loved one. I know all my help comes from above but it’s a blessing to know the angels I am surrounded with at this time. I visited my uncle Johnny Myers on Memorial Day and I must say this building has really changed! I notice the beauty from the front to the back! The lobby was decorated so beautiful! As I walked through! As I got closer to the back there was residents sitting in the day area enjoying the music and full of joy! The CNA was really involved with the residents! As I was leaving one of the residents said come back to see us with a smile on her face! Everyone seem to have been working hard and making sure the residents was happy! Seeing this made my Holiday very special! Keep us the good work at FAPA!

On May 14th I had the pleasure to visit FAPA, I was so amazed with the luau that they had for nursing home week. The residents and staff was so happy. The visitors including myself had a ball. The management team were very friendly. I even had the pleasure to visit Jojo at the Tiki bar. 😃 Thanks for your entertainment at the bar. I was visiting one of my old neighbors (Mr. Harris) and did not know that I was walking into this. Thanks for a great night!

I would never send my loved one here. It REEKED of urine when I walked in and immediately knew my father was in danger. He was in the hospital for 13 days and was discharged for rehab to re-learn walking as he is very weak. I walked in 7 hours after he was admitted here and found him completely neglected. He couldn’t breathe. His breathing was labored, he was disoriented, he was unsafe. There was no bedsheets, no diaper, sheets stained with urine or had holes in them, had no medicine administering, no blood sugar checks (he has diabetes), urine bag was overflowing, he was sweating with the air on 76. A small detail, but the TV was tiny. That’s all he has to look forward to because he cannot move himself. He was so disoriented from neglect that he was staring at the TV on the weather channel for hours. I pressed his call button and someone (not the nurse) came in after 45 min. In the 2 hours I was there, the nurse never came in. I asked for them to check his urine bag. She said her predecessor did not empty it as they should have. I asked why he had no diaper, why his bed did not have a sensor on it in case he got up and fell. She proceeded to say that it is considered a “restraint” and they cannot do that. I asked what would happen if he fell. She lowered his bed toward the ground and said “now if he falls it will only be about a foot.” I asked how often he was checked on and she said about every 3 hours. I asked where the nurse was and she said she didn’t know, she was making rounds, and they had 45 patients to attend to. Meanwhile, someone is screaming “help” from another room for an hour and a half with no one attending him. The poor man was probably sitting in his own feces for hours because it smelled as I walked by. The lady called it a “2 star hotel.” I asked my father if anyone had been in the room since he got there and he told me no. He had been there for 7 hours!!!!! Finally, she said she was calling the nurse because his breathing became worse and he could barely speak without wheezing. She checked his oxygen and it was in almost deadly levels. I had to go out into the hallway and take the cart myself back to monitor his levels because they said they’d be back in about an hour. I called a doctor and they told me he was being completely neglected and to call 911. I tried to be respectful and tell the lady working there that he needed to go to the hospital ASAP before I called 911. She said “let me text my boss, I have to give these people their medicine that was due at 9pm (it was 11:15pm). My father never received any medication. The Second Lady came in with oxygen and said “if he goes up he’s going to stay here.” I said absolutely not. As soon as she left the room, he ripped the oxygen out. I asked her to come back in. She said she didn’t have the capacity to monitor him to make sure it stayed in. Again, he was confused and didn’t know what was going on. She finally came back in and said she was calling EMS. The other sweet lady actually gave me a hug and said she was so sorry and grateful I came in that night because he was going to be safe because of me. He was transported to the hospital where he was so confused, he barely knew my name. The ER doctor said he was lucky to be alive. I am absolutely disgusted with the level of care here and am scared for other patients. I don’t know how this place has positive reviews. I’m so grateful my father is alive.

I really enjoyed my stay here. All the staff were easy to get along with. The therapy dept was amazing. The food was very great. The nursing staff was wonderful and took good care of me. The activities were on point. Thank you FAPA.

This is a very nice and clean facility. The staff was very nice and friendly. The therapy was awesome. The food was amazing. The ladies in the activity dept. kept me busy. Thanks for a great stay.

This is the second visit to this facility. The building needs some renovation due to the age of it but the care for my Aunt, Ethel Bennett has been phenomenal. Everyone from the front desk staff, therapists, CNAs, Nurses, Dietary, Housekeeping, beauty salon, etc. were great. My aunt wanted to shout out the names of her exceptional staff; Bernice, Crystal, Valarie, Cindy, Lucky,Billy, Josephine, Kalani, Amanda, Marcus, Molly, Melissa, Sharon. Thanks to Miss Sharon for the good food, she was always making sure my aunt had what she wanted to eat. Also shout out to ALL of her nurses who took care of her. Lastly, Dr Nora and the Nurse Practitioner came by everyday they were here to check on her.
